Before discharge

Get a written med list, mobility precautions, wound instructions, and follow-up appointments.


Home setup

Remove trip hazards, add nightlights, stock easy-to-prepare foods, and set up a main-floor rest area if stairs are tough.


First 72 hours

Confirm meds, hydration, and pain control; schedule transportation to follow-ups; keep a simple symptom log.


Support team

Blend family, private duty caregivers, and—if ordered—home health nursing or therapy.
